Pulmonary & Primary Care Associates staff have been trained to safeguard your privacy and we comply with federal privacy regulations. At the time of check-in, new patients must sign a form acknowledging receipt of our privacy notice. If you feel your privacy has been compromised, please call our practice administrator at 508-747-1318.

Medical Record Copies

Patient medical records are technically the property of PPCA and we are required to maintain the original record. We are not allowed to give out the original medical record, even to the patient. However, patients are always entitled to obtain a copy of their medical record. Please read our privacy notice for more details. If a patient requests a copy of her medical record, the first copy will be provided free of charge. Subsequent copies and all requests from attorneys and insurance companies will incur a copying fee. Patients must sign a medical record release form in order for us to send copies to anyone other than the patient.
